Community Health and Education
We help our community grow stronger and healthier by providing more than 10 ongoing programs that improve access to health care, encourage prevention and provide education for people of all ages. The community activities we offer or participate in each year include free or low-cost care, health lectures, exercise classes, support groups and flu-shot clinic.

In order to best serve our community, please submit your feedback through the Community Health Needs Assessment. The survey takes approximately 10 minutes from start to finish.

Participate in the Community Health Survey

We’d love to hear from you. Please complete the Community Health Survey using one of the methods below. This survey is completely anonymous (you remain un-named and unknown to us) and confidential (all information collected is private and only shared as group summary).

The community survey will be available for all residents, 18 years of age and older, living in Lincoln county.
